Wellness Exams and Comprehensive Physical Exams Self-Pay Packages Female Wellness Physical Exam Package  -  $141 (includes lab fees except pap smear****) Includes:  -Consultation with one of our physicians -Review of medical history -Height, weight, body mass index (BMI) calculation -Blood pressure measurement -Routine physical exam -Female physical exam including pelvic exam, breast exam and                     breast self-exam instruction -Skin examination and counseling on skin cancer prevention -Review and explanation of lab results -Contraception management (if applicable)* -Referral for mammogram (if applicable)** -Routine lab testing*** -Lipid panel (cholesterol test) -Comprehensive metabolic panel (blood sugar, kidney and liver function panel) -Complete blood count -Pap Smear (Please note that Pap Smear pathology charge will be billed seperately and is                     not included in the above fee)**** Male Wellness Physical Exam Package - $141 (includes lab fees) Includes:  -Consultation with one of our physicians -Review of medical history -Height, weight, body mass index (BMI) calculation -Blood pressure measurement -Routine physical exam -Skin examination and counseling on skin cancer prevention -Review and explanation of lab results -Routine lab testing that includes*** -Lipid panel (cholesterol test) -Comprehensive metabolic panel (blood sugar, kidney and liver function panel) -Complete blood count Comprehensive Physical Exam - Starting at $130 Includes:  -Consultation with one of our physicians -Review of medical history and medications taken -Review of immunization history -Review of risk factors (cardiovascular risk, cancer risk, etc) -Review of life style recommendations (diet, exercise, tobacco cessation, etc) -Height, weight, body mass index (BMI) calculation -Blood pressure measurement -Comprehensive physical exam -If female:includes pelvic examination, breast exam and breast self-exam instruction -Skin examination and counseling on skin cancer prevention -Review and explanation of lab results (if applicable) -Referral for preventative medicine testing if appropriate (such as colonoscopy,                     mammogram, etc) * Includes a prescription for oral, transdermal, injectable or vaginal ring contraception if desired by patient and there is no medical contraindication ** Referrals for mammograms are made to a radiology facility.  Mammogram testing fees will be charged by radiology facility. *** Additional testing can be done at an additional charge **** Due to government regulations, all pathology charges (including the Pap Smear) need to be billed directly to the patient.  South Denver Primary Care will not bill our female patients for Pap Smear pathology charges.  Femaie patients will receive a bill from our pathology provider for all Pap Smear patholgy services. The above pricing is subject to change at any time without notice.  Check this web page the day of your appointment for current pricing.   Frequently Asked Questions About The Wellness Physical Exam And The Comprehensive Physical Exam 1.)  What else do I need to know about The Wellness Physical Exam and the Comprehensive Physical Exam? To be eligible for the Female or Male Wellness Exam Packages you must be between the ages of 18-49 years old.  If you are 50 years old or older, we are only able to offer the Comprehensive Physical Exam below.  Evaluation of acute medical issues and chronic medical diseases is not included as part of this wellness package.  Procedures (other than the Pap Smear) are not included as part of this wellness package. Note Regarding the Comprehensive Physical Exam: All lab fees are an additional charge.  Click here to see the Self-Pay service rates for common lab testing.  Evaluation of acute medical issues and chronic medical diseases is not included as part of the Comprehensive Physical Exam.  Procedures (other than the Pap Smear) are not included as part of the Comprehensive Physical Exam. 2.)  I am interested in scheduling an appointment for a Wellness Physical Exam.  How do I make that appointment?  When you call us to request an appointment for a Wellness Exam.  You need to be fasting for 12 hours prior to your appointment.  You can drink water but do not consume anything with calories 12 hours before the blood draw appointment.  Payment is due in full at your appointment.  We accept cash or VISA/Mastercard.  3.)  I am interested in scheduling an appointment for a Comprehensive Physical Exam.  How do I make that appointment?  Call us to schedule this type of exam.  You need to be fasting for 12 hours prior to this appointment.  You can drink water but do not consume anything with calories 12 hours before your appointment.   4.)  What is the difference between a Wellness Physical Exam and a Comprehensive Physical Exam?   The Comprehensive Exam is similar to a typical “Annual Physical” most adults receive once a year.  During a Comprehensive Physical Exam, our providers take a more detailed look at your history, they review your risk factors and immunization history, and they perform a more detailed examination.  5.)  What is not included in the Wellness Physical Exam and the Comprehensive Physical Exam?  Evaluation of acute and chronic medical issues and procedures are not included as part of these physical exams.  If you have an acute medical issue, it is best to schedule an acute visit with us before you schedule a physical.  If you have a chronic medical problem (such as diabetes, depression, high cholesterol, etc), the provider will not be able to directly address this issue at your physical.  You will need to schedule a separate appointment to address these types of concerns.  Charges for additional lab work and immunizations are not included in the above exams. 6.)  I would like to make an appointment to only have a Pap Smear done.  What would be the cost of this?   If you are a Self-Pay patient and you wish to have medical services provided other than those listed above, you can make an appointment for a consultation.  Click here  to see our Self-Pay consultation and lab fees.
